Over the years, we have seen that patients and families with regular and structured care typically have the best outcome and experience. It is very important that patients seeking individualized care are seen on a regular basis to ensure efficient progress. Operating as a membership-based private practice has allowed us to limit our practice size and allow for a more personalized, caring experience for our patients.
Pediatric Membership Plans
The membership price per year varies based on the number of well-child appointments required, which decreases with age. Below you will find the price for each year based on age. Those joining the practice in between our membership years (for example, joining at 6 months or 18 months, etc.) will be prorated to a customized package for the remainder of that age year.
Our pediatric membership plans include:
Membership Dues for the year of service that include all membership benefits listed in that section.
In-office screenings for: anemia (Hgb), urinalysis (U/A), eye exam, hearing screen with each well-child exam.
All necessary well-child exams that will take place during each year, as listed below:
Newborn (0-1) Well-Child Exams Due at: 2-4 days, 7-10 days, 2-3 Weeks, 1 mo., 2 mo., 4 mo., 6 mo., 9 months.
Year 1-2 Well-Child Exams Due at: 12 months, 15 months, 18 months, with Hemoglobin checks included.
Year 2-3 Well-Child Exams Due at: 24 months, 30 months, then switch to annual health maintenance exams.
Ages 3 and up require one annual well-child exam, and switch to a health maintenance, calendar year plan.

In-house labs outside of those planned for each well-visit.
Additional appointments, outside of the wellness exams included in the membership package, are discounted for members, ranging from $160 - $320 depending on the length and complexity of the visit.
Telehealth and phone consults are available for certain appointments and are also at discounted rates.
Health Maintenance Membership
The Health Maintenance Membership is for all patients ages 3 and up and is done by calendar year. The annual fee is $1,030 for the first year, due to a one-time new member fee, then $680 for subsequent years. The membership covers the cost of an annual wellness visit. This can be a well-woman exam, or a functional medicine visit for adults, a well-child exam with all recommended testing for children ages 3 and up. In addition to the annual wellness visit, the health maintenance membership includes a Dues cost, which includes the following:

Authorizations, Prescription and supplement facilitation, test ordering outside of office visits, coordination of care with specialists, and other behind the scenes services that other offices may charge for individually, are included for members.
A Functional Medicine approach which is more thorough and promotes overall wellness.
Access to Home Visits for those who are in the area ($450-$650/visit).
Smaller practice size to allow for reasonable and urgent appointment scheduling.
Patient Portal and Communication Access 24 hours a day.
Significantly Discounted rates for all visits needed outside of the included wellness exams.
Member visits range from $160 - $320 depending on the length and complexity of the visit.
Non-member visits are rated at $500/hour and do not include all of the same benefits listed here.
Health Optimization Membership
Patients who have a desire to embark on a profound look into their health and wellbeing may upgrade to the Health Optimization Membership Plan, an intensive two to six month healthcare plan that will be tailored to your needs. After the program's completion, patients will have the option to renew their Health Optimization Plan or remain an active patient via the Health Maintenance Membership. Further details regarding this membership can be discussed during your new-patient appointment.
